Application Forms
New Members! Fill out a new member application (located outside the Student Organizations Office - Green 51). Return completed application and receipt of payment to the Student Organizations Office (Green 51) prior to the application deadline. A one-time payment of $75.00 must be made at the Accounting Fee Window (Green 66).
